The position for which I am being considered requires me to consent to a criminal background check as a volunteer. In connection with my services with Victory - A Church of His Presence, I hereby authorize the church to initiate a criminal background check on my behalf. I understand that this check will cover a search of law enforcement and court records and a check of National Sex Offender Public Registry.
I understand that my ability to serve as a volunteer with Victory is contingent upon the results of the background check. In cases where a potential volunteer has a criminal record, Victory’s executive staff looks at a number of criteria when determining whether or not to refer an individual to a ministry team/position, such as the nature of the crime, the number of crimes, when the crimes occurred and more.
I understand that failure on my part to complete the criminal background check will result in the revocation of any position offered to me or accepted by me. I acknowledge that the information from the background check may be shared discreetly with other ministry leaders related to my volunteer position. I make this authorization to be valid for as long as I am an applicant or volunteer with Victory.
I certify that the information provided above is truthful and accurate to the best of my knowledge. I understand that knowingly providing false information or omitting information may result in my disqualification or termination from a volunteer position at Victory. I understand that my social security number will be required to obtain a security background check.

You will be required to provide Backgroundchecks.com with your social security number, along with other information. Each background check performed is $12.00. Though not mandatory, we ask you to consider contributing to the cost of your background check to help offset the costs of these reports.
